Saving Talk Therapy: How Health Insurers, Big Pharma, and Slanted Science are Ruining Good Mental Health Care, by Enrico Gnaulati

Review

“Saving Talk Therapy is a compelling, meticulously researched, and accessible account of how the machinations of two very powerful industries—health insurers and pharmaceutical companies—have undermined the viability of the practice of talk therapy and corrupted the research paradigms in use to evaluate the effects of all mental health treatments. Moving beyond critique, Gnaulati provides clinical vignettes from his own practice to illustrate what is beneficial about psychodynamic/humanistic psychotherapy. For the millions of mental health patients who are dissatisfied with the dehumanized treatments they have received, and for the thousands of young mental health professionals who know the work they are doing feels much less rewarding than they had thought it would be, this is the book to read.”—Ronald B. Miller, professor of psychology, St. Michael’s College, and author of Not So Abnormal Psychology: A Pragmatic View of Mental Illness“In this remarkable and highly readable book, Dr. Enrico Gnaulati, an eminent author and seasoned clinical psychologist, provides a compelling and spirited argument for the value of psychotherapy for individuals and for society. Drawing upon a wide range of research, he also does the public and mental health professions a great service by questioning the often extravagant claims made for the effectiveness and safety of psychiatric medication, and the overstated benefits of quick-fix therapies.”—Steen Halling, professor of psychology emeritus, Seattle University, and author of Intimacy, Transcendence, and Psychology: Closeness and Openness in Everyday Life“Saving Talk Therapy is an impassioned defense of the emotionally evocative form of psychotherapy that many people still seek. It is now vanishing. Dr. Gnaulati brilliantly lays out the history of the field and provides incisive analysis of the forces—political, economic, and cultural—that have endangered its future. This is a compelling and essential read for mental health professionals and consumers alike.”—Ben Gorvine, assistant chair, Department of Psychology, Northwestern University“Saving Talk Therapy is a brilliant exposé of what has happened to the field of psychotherapy and should be required reading for therapists and patients alike. Both authoritative and engaging, Dr. Gnaulati masterfully lays bare the deceits and disinformation that prevent most people from ever getting meaningful therapy and most therapists from learning how to provide it.”—Jonathan Shedler, PhD, Clinical Associate Professor of Psychiatry, University of Colorado School of Medicine and author of The Efficacy of Psychodynamic Psychotherapy

This is a wonderful book, written by a thoughtful and insightful psychotherapist.I am a psychologist and psychotherapist, I identify as primarily humanistic and secondarily psychodynamic, and agree with the majority of the author's thoughts and conclusions. It is really difficult to find psychotherapists who practice real talk therapy, related to a variety of factors that the author outlines.Because I agree so much with what the author has presented, I will focus on some areas I feel he neglected, or was not up to date. The author criticizes the current overfocus on CBT and short, structured treatments. Working at the VA, however, I have observed that a large minority of patients actually improve significantly with these treatments. When they do improve, they note that they have learned coping skills that help them feel in control of their symptoms, whereas before their symptoms were uncontrollable. Also, some patients want to work in a brief and problem-solving manner, they don't want, or need, extended psychotherapy, although this could be helpful in areas beyond symptom management. So, CBT is very useful for some patients.Which brings me to the central issue the author missed, in my opinion. Psychological treatment is beginning to fragment into 1) treatment of diagnosable mental disorders, which is short term and structured and paid for by insurance companies, and 2) psychotherapy for the sorts of issues described by the author, the problems of living, sometimes severe, that all of us have, to a greater or lesser extent, including crises that precipitate a desire to enter psychotherapy. The author is advocating #2, as am I. And much of #1 can be treated in the second matter, and often more profoundly and definitively. However, this longer, less structured, open ended psychotherapy is no longer being paid for by insurance companies, and unfortunately, organized psychology has colluded in it's demise with the overfocus on diagnosable mental disorders treated in short term, structured, time-limited therapy, the so called "evidence based treatments."So we are talking apples and oranges and I see no way that this is going to change. It is heartbreaking. I see it every day at the VA. Patients who need longer term care, including for ptsd, and are terminated from treatment because they have reached "maximum treatment benefit" (i.e., a certain number of sessions). Fortunately, I work in a spinal cord injury center and these same constraints are not placed on my work, but I am very aware of what is going around me and how a large number of veterans (and in the non-VA world, regular citizens) have no access to the help that they really need - unless they have significant financial resources. In my mind, then, talk therapy is dying, and there is no stopping it, just as psychodynamic therapy has been replaced by cognitive-behavioral therapy. Perhaps there will be a small number of psychotherapists from various disciplines who will be able to make a go of being talk therapists, but this will not be the norm. It is incredibly sad to me, knowing how transformative 'real" psychotherapy can be.

Having been in practice over 40 years, I was very pleased to read Enrico's reminder of the importance of the humanistic and depth approaches to the practice of psychotherapy. In the future, (which is rapidly approaching) an informed culture will demand that this type of therapy be the "standard" for truly helping our patients and the society as a whole. This is, and should be, a must-read for mental health professionals and patients alike. One cannot overstate the usefulness of active engagement, authenticity and the judicious use of humor in helping people.

Gnaulati has written a valuable account of how talk therapy has been methodically misrepresented as not supported by research and set aside for short-term, manualized therapies that don't actually live up to their "evidence-based" claims. He explains clearly the research conundrum, the forces that prefer to push talk therapy to the periphery, and describes the evidence for these therapies of depth and relationship. An important book that's a wakeup call about a mental health system crisis that is growing in the U.S.

As a practicing psychiatrist, I trained in providing various forms of supportive and insight-oriented therapy. Each of my training sessions with patients was an hour long and I quickly learned what this author also learned through his experience - simply providing medication doesn't work well, and patients often seek more feedback than simple reflection of their own statements. Therapy is very much a two way street. The author describes what has happened, and what has gone wrong, with a system driven by research - research which can't do a very good job over a years-long therapeutic alliance to determine the benefits of such an alliance. Unfortunately, we're now stuck in that system and the only way out for those of us in the field is to have our own private practice and to charge fees directly of the patient, bypassing insurance in the process entirely. That works, but it also means that a good proportion of the public can't afford the care that would actually result in lowering of their overall demand on the healthcare system. If only the legislators would read this text, perhaps we'd all have a chance.

From Publishers Weekly

Readers familiar with Fraser's work in Gourmet, O the Oprah Magazine, More, and other publications may recognize the author's efforts at self-improvement, attempts at finding love, or meals she's had in places like Lima, Peru: "We try ceviche of wild sea bass with lime and red onions... we have spicy chifa food in a downtown Chinese restaurant... we stuff ourselves with stuffed peppers..." In following up An Italian Affair, Fraser takes a closer look at her own wanderlust and examines the positive and negative effects it has had on her life, particularly over the past 10 years. For example, the author finds a glorious waterfall in Samoa. "The water is so clear light blue it's possible to see all the way to the bottom, and the bottom is a long way down. I dive in for a swim... this is why I love to travel." But she questions the notion of having it all, or having it all at once, and worries that her desire to explore and her professional success have come at the expense of stability and family. She challenges the ideals of happiness and home she had previously held, adding a layer of depth to a memoir that will excite travelers of the world and the armchair alike. From Booklist

Travel writer Fraser enchanted readers with her tale of her postdivorce romance with a sophisticated French professor in An Italian Affair (2001). She continues the story here, beginning with the end of the affair after the professor finds someone he wants to settle down with. Fraser wants the same thing, but she still finds herself most drawn to free-spirited men looking for flings, not relationships. An assault in Samoa leaves her shaken—and afraid to travel alone. Fraser turns her focus inward, trying to find peace through meditation and to temper her impulsiveness. Her forty-fifth birthday brings the end of a relationship and the beginning of something new when Fraser travels to the Mexican city of San Miguel de Allende and finds herself contemplating buying a house. The title is an apt description of both Fraser’s travels—which include jaunts to Italy, Provence, and Rwanda described in evocative, lush prose—and her frame of mind over the course of the eight years that her winning coming-of-middle-age memoir spans. --Kristine Huntley

This is truly an enchanting book and will take you to a place outside of the world you normally livei n. There's something relatable, yet not, about this incredible book. It's truly a coming of age story that hits close to home. I loved this book and thought it had everything you needed in ones life - at least as a strong independent woman with a free spirit. This book definitely was a kindred spirit for me. The authors writing makes you forget you are reading a book, it's hard to put down.

I always enjoy Laura's writing. She comes across as warm and personable. I did get tired of her complaints about being 40! She is healthy and energetic. A lot of people never even get there! I thought the forties were sexy and fabulous. I'd love to be 40 again! If she wanted to start a family she could have been more selective in her choices of men. She could always adopt, people have adopted in their 60's. Still I admire her honesty and adventurous spirit.

I got this as a Kindle deal in December, and really enjoyed it. I marveled at how much of the world this woman got to see as a freelance writer living in San Francisco. Certainly something to congratulate her on. I enjoyed the travel writing and the personal ruminations. Some people think she came of as whiny. She didn't. She just wore her thinking cap throughout and talked about her weaknesses candidly. The important thing is that she grows as a person, and illuminates the challenges and rewards of her journey for us. A nice memoir, well written.

I gave this book 3 stars because I think future readers should not choose this book in anticipation of another "An Italian Affair". I did and I was quite disappointed after reading the first few chapters or so. Unlike her previous book, miss Fraser almost focused entirely on her own internal struggle, her race against her own clock, her effort to overcome her midlife crisis, wherever she visited, and thus in my opinion failed to capture the readers' interest with her travel. I found myself racing through the pages many times to fast forward to her next trip.On the other hand, this book was quite easy to read. It was honest, unpretentious, and quite revealing. In fact, it may be a bit too honest and revealing for a male reader such as myself. (Perhaps the female readers would think otherwise.) However, I must say that her writing in this book is much more eloquent and profound comparing to "An Italian Affair".To borrow Lillian Hellman's words, I think miss Fraser "left too much of [her] unfinished because [she] wasted too much time" looking for someone to make her life complete. The last chapter however was very poignant and in my personal opinion brought some closure to miss Fraser. At least that was how I felt at the end.I finished reading "All Over The Map" despite many times I wondered if I had chosen the wrong book. In the end, I did like the book for what it was. It just was not what I had looked for in the beginning.

I guess if you are a middle aged woman struggling with being middle aged this book could be amusing. The author does share some good humor.

This book stands well on it's own as a travel / memoir but it is best read and understood once you've read Fraser's "An Italian Affair" because the Professor is still the most compelling human character.

I read this book right after I finished "An Italian Affair" (which I also loved). I really enjoy traveling but I don't usually like travel books. This book is the perfect combination of a good romantic, adventurous, and informative story wrapped up in one. It also is a very honest look at a woman's life....whether you are single, married, with or without kids, I think everyone can find something that will touch them. I came away from it thinking that there is no perfect answer to anyones lives. We must all live our lives the way that fits best. Things can change along the way and as long as you are willing to go with those changes and not try to stick to what your original plan was, you'll have an interesting life. Laura Fraser has had one interesting life that I am grateful to have been able to experience through her book.

I love that this book is about a person who doesn't weigh her happiness on the Prada handbag scale. I wish I had a Prada handbag...It is refreshing to read a book about a strong, single woman in search of herself and the rest of the world. I thought that this book was funny, witty, touching and made me want to book my next vacation... which I then did.

I, like most Americans, am obsessed with "celebrity". I read the entertainment magazines to see pictures of the elusive star, both shopping at the market and on the red carpet. Vial captures these men in one of the most intimate places; our bedroom, a place reserved only for lovers and the men's children. The men, some unrecognizable by myself, range the gamut. From the uber-intimate (James Franco sitting on the toilet), the comical (Dermot Mulroney cradling a snarling dog) and the sweet (Ethan Embry holding a baby). From their gestures, environments and faces, we hopefully get a better sense of who these men are when the cameras aren't rolling. Their actions are what our men do every day, in this book they're "just another guy." And that's the genius behind Vial's idea; photographing well-known people (who most of us will never meet) doing the things we do; therefore establishing a connection between the every-man and the superstar. This book is suited for anyone interested in Hollywood culture.

No matter what celebrity drew you to this listing, you should know that this book is well worth having. Any star photographer can assemble a collection of famous people, but only Veronique Vial can bring you something quite like this. What you see, as you look at these images, are people so completely at ease with her -- and the camera's -- presence that they've let their guards down and revealed their most basic, human selves to her, to her camera, and to us. These are the stars before they put their public masks on, and Veronique treats them with enormous respect and kindness, while treating her audience to something we otherwise can never have: an intimate, private moment with our favorite public figures.

I found quite a few images very interesting. This is not your usual male art book. But I do Think it is worth having !

When this book came out about 8 years ago, I was immediately enamored. It's amazing, because even relatively unattractive men are captured in a very flattering light. And the attractive men are captured in a VERY VERY flattering light. This book is so much more interesting than what I remember. Please note: this photo collection will not be of any interest to people who are not interested in looking at pictures of (sometimes half-nude) men.

This book isn't worth the money. The only one in it that looks great is Vin Diesel. The rest of the men, well what can you say. I wish that I had not bought the book. I was sadly dissapointed with it.

not as interesting as the women version.

The book arrived in horrific shape - the binder was torn from the pages and many of the pages were stained with a white crusty substance.I was INCREDIBLY disappointed.

Ah. I really know nothing of photography but most of these pictures are at least *ahem* nice to look at. I would have preferred a little blurb from author regarding the taking of every picture (sometimes there is an explanation of a picture, but mostly not), and even more so HOW exactly she got permission from all these men to take pictures of them in bed, in front of the mirror, at breakfast, in the shower, on the toilet (can you imagine though? "Hey let me come to your house and take pictures of you when you probably look your worst, it's for a book. Come on, it'll be awesome."). But yeah, really the thing I find most interesting is finding pictures of actors I now know as fairly famous before it was common to know their name (there's a picture of Adam Scott in here and despite watching Parks and Rec on a regular basis I didn't even recognize him).
